Our online consultations
Accessing weight loss treatment with Align Weight loss requires you to complete an online consultation where you answer questions about your health and medical history. You must complete the online consultation completely, truthfully and to the best of your knowledge. The online consultation must be completed by you personally. If you are unsure of the answer to a medical question, then please contact your GP to discuss your medical history. The answers to your questionnaire form the basis of your medical consultation and assessment for your suitability for Align’s weight loss programme.
If you need help accessing or completing the questionnaire, e.g. because you have a disability, please contact us and we will assist you with it. If you need help understanding a question we have asked please contact us at info@alingmedical.com
In line with the General Medical Council’s advice set out in Good Medical Practice, we will seek to inform your GP about the medical treatment you undertake with Align and the medication you are prescribed. This is important to ensure your medical record is complete. You may refuse consent for us to contact your GP.
Your medical record
When you sign up to services with Align Weight Loss you agree to us creating an electronic medical record (EMR). This record is held securely and will not be shared with any third party. The record will contain your personal details, your medical history as is relevant to our service and details of your treatments with us. In line with best practice for medical record keeping, you acknowledge and agree that we may archive your EMR including your personal information and treatments for a minimum of 8 years following your last order or prescription.
You are responsible for providing us with up to date and accurate medical information. If there are any changes in your health (new diagnosis, new medication, etc.) you must let us know as soon as possible. Please contact us at info@alignweightloss.co.uk or send a message to your Health Coach advising that you need to let us know about a change in your health. We will then arrange for a doctor to get in touch with you to discuss this. Any updates in your medical situation could affect your suitability for treatment.
You are responsible for maintaining the confidentiality of your account and password in order to keep your account secure. Please inform us if you believe your account has been compromised. You must not register more than one account with Align Weight Loss.
ID verification
In order to safely prescribe prescription medications, we must verify your identity. This will involve you sending us a copy of your photo identification e.g. passport, driving licence or other nationally accepted ID (e.g. Military ID).
